☑ Are 50 years of age or older

☑ Have had a heart attack or stroke — or have been told by a doctor you're at risk for one

☑ Have had a heart stent placed or heart bypass surgery

☑ Have been diagnosed with chronic kidney disease

☑ Are able to commute to Sarnia, ON for study appointments
